Apply for Membership

Complete the following form to apply for adult membership of ACREM. If you are wanting to apply for Cadet membership (i.e. under the age of 16), please use the Cadet Application form.

NOTE: The following questions are applicable to applicants for Monitor or Reserve Monitor membership only. Applicants for Associate membership do NOT need to complete the following questions and may skip to the next section (Associate applicants may answer these question if they wish, but it is only mandatory for Monitor/Reserve applicants).


Criminal History

Other than minor traffic and/or parking infringements, have you:

  • in the past 5 years been found guilty of any offence with no conviction recorded; OR
  • in the past 5 years been convicted of any offence where a fine or period of probation, supervision or community service was imposed as the penalty; OR
  • in the past 10 years been convicted of any offence where a term of imprisonment was imposed as the penalty.

in any Australian state or territory , or overseas?

NOTE: A criminal history does NOT automatically preclude you from membership. You will be given the opportunity to explain the circumstances and a decision will be made based on all the facts, including the time since last conviction, type of offence, and the circumstances surrounding the offence. However, failure to disclose any relevant criminal history will be grounds for the immediate refusal of an application, or the immediate dismissal of a member.

Working With Children

Various Australian legislation makes it an offence for a prohibited person (a person convicted of a serious sex offence, the murder of a child, or a child-related personal violence offence, as well as a person classed as a registrable person under relevant legislation) to apply or otherwise attempt to obtain, undertake or remain in, child-related employment, unless an order has been made by an appropriate authority that the particular offence does not apply.

Legislation governing these matters varies from state to state, however generally a prohibited person is considered to be a person that has:

  • a conviction for a serious sex offence (including but not limited to, sexual assault, acts of indecency, child pornography, child prostitution and carnal knowledge); or
  • a child related personal violence offence (including but not limited to, intentionally wounding or causing grievous bodily harm to a child); or
  • a conviction for assault, ill treatment or neglect of, or psychological harm to, a child.

ACREM Members that undertake active field work, including publicity events, may be placed in situations where they are responsible for the care of a child, especially when working with other groups or our Cadets. As such ALL applicants (and current members) must complete a declaration that confirms they are not a prohibited person. Some members may also need to undergo a WWC criminal history check, however if this is necessary the relevant forms and additional information will be posted to you prior to any checks being conducted.

I am aware that I am ineligible to apply for or otherwise attempt to obtain, undertake or remain in, child-related employment if I have been convicted of a serious sex offence or child-related personal violence offence as defined in the NSW Commission for Children and Young People Act 1998, or if I am a Registrable Person under the NSW Child Protection (Offenders Registration) Act 2000 (or such other relevant legislation). I have read and understood the above information. I am aware that it is an offence to make a false statement on this form.
